User :   Dear Doctor
My mum was treated for Breast CA approx 3 years ago. She is now 61 years, she is diabetic and was initially taking Nolvadex (tamoxifen) for 2 years. Following this she started with FEMARA but after 3 months there was terrible itching and the doctor advised her to discontinue and she was advised to take ANABREZ which she started since 2 months approx. My mother has been diabetic since 15 years and was always taking Glucophage (1000mg) once a day but now since a month her sugar levels have gone really high and she has been advised to take 2 doses of Insulin injection morning & evening and also Glucophage in the afternoon in spite of this her sugar level today is 164. I am not too happy with her change of medication to Anabrez and I would like ur advise please
Doctor :   Hi
Doctor :
Let me read it
User :   thanks
Doctor :
Ok
Doctor :
Now wrt her change in medication, how is she with Anabrez?
User :   Well the big problem which I see now is her high sugar levels and sudden dependance on Insulin which was not the case all these years.
Doctor :   Ok

Doctor :
the 161 reading is in the fasting state or after food?
User :   I took this as soon as she woke up this morning..I guess fasting state
Doctor :
Ok
User :   Her diabetes was well controlled even when she was on FEmara or tamoxifen
Doctor :
How are the levels after food?
User :   I have not checked
Doctor :   Ok

User :   I am thinking of checking again tomorrow fasting
Doctor :
If it is maintained around 200 it is good
Doctor :
the post prandial ones I mean
User :   Doctor is there anything else she can go instead of Anabrez...
Doctor :
as very strict control is not a recommended practice now
User :   I dont understand what is Post prandial
Doctor :
2 hours after food intake
Doctor :
Do it after her 3 meals as well
User :   So u mean I should check her sugar levels 2 hours after a meal?
Doctor :
like one you can do today after breakfast
Doctor :
tomorrow after her lunch
Doctor :
and day after her dinner
Doctor :
like that
Doctor :
though not on the same day
Doctor :
It is 2 hours after the meal
User :   and what would u recommend I do if her sugar levels are not down....I mean I dont see any reason why she has to be insulin dependant all of a sudden
Doctor :   Ok

Doctor :
She has been given certain drugs
Doctor :
which fluctuates her hormonal levels
User :   I hope I am chatting with an Oncologist ?
