User :   hello doctor. I have a few questions
User :   first off i had surgery to check for endometriosis back in jan 11 of this year

User :   i've been bleeding ever since then and I want to know if this is normal to be bleeding for this long

May I know what surgical procedure you have been through?
User :   at first it started out as spotting after the surgery, then after a few days started like a full blown period. and has lasted this long.
User :   they did a laproscopic procedure
User :   checking for endometriosis

Is the bleeding severe as the first day or is it reducing now?
User :   it's worse than when it first started but not any heavier than a regular period
Doctor :  
Ok.
User :   my concern is that it's been going on for a few weeks now
User :   i would think it only to be a period,but I have a IUD
Doctor :  
Usually healing in a surgical procedure heal in a few weeks to few months.
Doctor :  
It can take about 2 to 3 months time in certain cases.

So the bleeding can be normal.
Doctor :   If it is reducing in intensity and you do not have pain.\
User :   i do have cramping still

Then you need not be concerned.
Doctor :   Are you on medications?
User :   prozac, and dilaudid only when needed for abdominal pain, and reglan

You can get a physical examination by your doctor to check if there it is healing normally or if there are signs of infection to confirm this.

Though it is not an emergency.

User :   my post op exam is next week

You can wait till then, and need not worry.
User :   after the surgery they said i'd be spotting a few days...i didn't know it would be weeks

As I mentioned in some cases the healing may take a few weeks.
User :   ok
User :   another question...and this could be a broad range of answers i understand

If the bleeding is not heavy, it is not of concern.
User :   i had this surgery for them to check for endometriosis and explore for any other reasons why i've been having chronic pain for the past 3 years in the RUQ of my abdomen

User :   they said they noticed some scar tissue where my gallbladder had been (removed in 07) but wasn't enough to be causing the pain

User :   they ended up taking a biopsy and i guess that it showed ongoing inflammation

User :   do you know what could be causing that? and if that could be causing my pain?

Can you describe the nature of the pain?

I mean is it a spasmodic pain?
Doctor :  
Or just a ache?
User :   it sharp feels like someone is stabbing me
User :   seems to always be there...however i've grown some tolerance and able to live with it most days

User :   no
User :   i had my gallbladder out in 2007 jan
User :   and it's been an ongoing problem since then

User :   no

User :   very rarely

User :   maybe 2 times a month at most
User :   liver enzymes are always normal...well lately they have been
User :   i've had 4 ercps and 2 sphincterotomies of my bile duct
User :   but those didn't seem to help at all

May I know if the biopsy was done on the skin scar or intraabdominal site where the surgery was done.

Have you checked your liver enzymes?
User :   i've had my liver enzymes checked yes. they are normal as of a few days ago
User :   the biopsy was from inside
User :   the ruq of my abdomen where i am having pain

I understand it is in the RUQ, but what biopsy was done. Are you referring to the skin scar inflammation?
User :   no
Doctor :   Ok.
User :   when they checked for endometriosis they also did a mini exploratory to see if they could visualize anything that could be causing the pain
Doctor :   In a gallbladder surgery the gall bladder is removed while the ducts are left behind.
Doctor :  
Inflammation could be acute or chronic.
Doctor :  
if the inflammation was an acute it could have been caused be a minor viral infection.
Doctor :  
however chronic inflammation could have been caused by a persistent infection as in hepatitis.
User :   been checked
User :   is it possible for a false negative results for hepatitis?
Doctor :  
Since your liver enzymes have been normal, there are nore possibilities of it being an acute problem.
Doctor :   HBsAg is fairly acurate and have very few false reports.
Doctor :  
So your symptoms may be related to gastritis or secondary to an acute problem related to minor injuries secondary to alcohol consumption.
Doctor :  
A detailed evaluation may be needed if the pains continues.

User :   i've seen gastroenterologists, biliary specialists, all kinds of people
User :   i'm kind of hoping that the biopsy results will helppoint me in the right direction

May I know what was the opinion of the gastroenterologist?
User :   they don't know
Doctor :   How frequently do you have the pain?
User :   everyday
Doctor :   Is it related to food intake - I mean does it increase when you are hungry?
User :   no
Doctor :  
May I know your occupation?
User :   i am a student but i haven't been able to work or go to school lately because of this
Doctor :   Ok.
Doctor :   Since you have had gall bladder removed, the other causes for the pain are two.
Doctor :  
1 would be due to gastritis.
Doctor :  
2. Due to hepatitis.
User :   ok
Doctor :   It also could be related to the stress and anxiety to.
User :   ok
Doctor :  
But only an examination and evaluation would be able to differentiate these.
Doctor :   You can get a liver enzymes checked (if it was not done recently) and if it turn out to be normal.
Doctor :   You may try consultation with a psychologist to check if stress has a role in causing the pain.
