User :   Hello. I need to consult a Urologist
Doctor :   I am a General Practitioner, I believe I can help you within my expertise and If required I shall recommend
a specialist
Doctor :   Hi Dr XXXXXXXX here
Doctor :   How can I help You?
User :   I am a 29 Year old Married Man. I have been diagnosed with azoospermia.
Doctor :   Ok
User :   The doctor at the fertility clinic confirmed this condition after 2 semen analysis in which there was no sperm detected
Doctor :   Ok
User :   She has recommended a testicular biopsy to further determine if my testis produces sperms or not
Doctor :   Ok
User :   There were FSH & Testosterone test also conducted for the same. The FSH report was 10.54 mg/ml
User :   the testosterone report was 3.33 ng/ml
Doctor :   Ok
User :   About 5 years ago i had a major urine infection since i had this habit of somehow not taking the leak if i was travelling.
User :   At that time of Urine infection i had started bleeding from my penis and it was very painful.
Doctor :   Ok
User :   The doctor at that time had advised me some tonic to be mixed with water and take it. The infection was gone.(I assume)
User :   but even now when i pee there is a certain amount of burning sensation which i am thinking is a result of the urinary tract being infected
Doctor :   Ok
User :   Is there a way that azoospermia in my case is because of this urinary tract infection and can this be cured by ayurvedic medicines like Mucuna pruriens and Tribulus Terrestris
User :   What do you have to say about my FSH report & testosterone report ?
Doctor :   Have You shown them to Your doctor?
User :   yes and she has told me that there is no chance that you can father a child naturally. she has completely ruled it out and told me that i might not even be producing sperms
Doctor :   Ok I think that It would be best If determined Whether You are still having the urinary tract infection
User :   What are the tests needed for determining the urinary tract infection ?
Doctor :   The same test which You did which told You that You had an infection
Doctor :   You would need to do a repeat test
User :   I still have a burning sensation in the urinary tract when i pee. specially when my bladder is full.
Doctor :   infection can lead to azospermia
Doctor :   However it is not the end of the world
User :   if infection has lead to azoospermia then is it curable with the ayurvedic medicines?
Doctor :   I think other causes need to be ruled out as well
User :   Oh ! I was told by my doctor that i am visiting in the fertility clinic that its a FULL STOP for me.
User :   She was saying the only way its possible now is a donor sperm and all that.
Doctor :   I would suggest that You go ahead with the testiculat biopsy
User :   can you please tell me how is a testicular biopsy done ?
Doctor :   it is a small procedure
User :   In both the semen analysis there were puss cells found in my semen sample. Can this be be because of the tract being infected ?
Doctor :   it is the best test for determining whether your testes is functional
Doctor :   it is possible
Doctor :   this takes arounfd 10 minutes
Doctor :   it is done under local anaesthesis
Doctor :   mybe a dull ache after the procedure which can be releived with painkillers
User :   and how much time is spent recuperating from the analysis
Doctor :   maybe around 2 or 3 days but You will be able to move about except for a dull pain
User :   Is it advisable to have a urine infection test before the biopsy ?
Doctor :   I do not see any harm in doing It
User :   Its just a urine test right ? I mean im talking about the urine infection test
Doctor :   It would be better if You treated Yout urinary tract infection completely
Doctor :   yes a urine test is so simple so You would lose nothing doiing it
User :   except my urine i guess
User :   :)
User :   just kidding.
User :   okay Dr. but the question is that since its been so long that i had this infection. starting the treatment now will solve it ?
Doctor :   Sometimes during the biopsy if sperms are foound the can harvest them and use them for intracytoplasmic sperm injestion
User :   but there is no way that an azoospermic person can father a child naturaly ? i mean it has to be a test tube procedure ?
Doctor :   Yes the possibility is less. However we need to see what exactly is the damage to Your testes thru biopsy and whether any sperms can be hervested from there to help you father a child
Doctor :   Yes it is a test tube procedure
Doctor :   The sperm onlyu if found will be taken and injected into the egg from the mother outside the body
User :   ok
Doctor :   So it all depends on Your biopsy
User :   but if say i have just a urinary tract infection and treat that. then will i be out of this azoospermia ?
Doctor :   I hope that the doctor who is doing You r biopsy is an expert
User :   I hope so too because I am just visiting a fertility clinic
Doctor :   No itf You treat Yout infection it does not mean that your azospermia problem will be solved
User :   Is it advisable to visit a different doctor for a second opinion ?
Doctor :   But if You take adequate treatment it will halt the damage to your testes
User :   ok
Doctor :   I think that Your doctor has suggesed a biopsy which is a correct evaluation
User :   ok
Doctor :   Like i said it determineds on the states of Your testis which we will come to know only biopsy
Doctor :   I wish You good luck
User :   Thanks
User :   Thanks for your time.
Doctor :   in case they manage to find a few sperms you can consider ivf treatment else it might not be possible to father a child
User :   which is the best place in XXXX for ivf treatment ? Will you be able to suggest ?
Doctor :   But i would suggest that You complete Your treatment for uti to halt further damage. However no guarentee that azospermia will disappear
User :   ok
User :   so you mean to say that even ayurvedic medicines are also not good for a permanent resolution to azoospermia ?
Doctor :   I would suggest that You first see results of biopsy whether the possibility exists or not
Doctor :   I am not familiar with ayervedic line of treatment
User :   ok
Doctor :   Please confirm Your number I will get customer care to give You a call
Doctor :   Hope You have understood?
User :   yes i have
Doctor :   Is there anything else You wish to know?
User :   not right now. thanks for your advice
Doctor :   Bye for now
User :   bye
Doctor :   Do give a good feedback
