User :   My friends son aged 20yrs met with an accident on 18.06.2010 . He was found to have
compound fracture left femur. He was operated at XXXX , bangalore and he is developed
RABDOMYOHLYSIS and acute RENAL failure and he was referred to XXXXX, bangalore for
further management after FASCIOTOMY , he underwent repeated wound dressings and
deberidements . On last debridement it was found that a part from lateral and mediacl
compartments, even the posterior compartment was completly necrosed.Keeping in mind the post renal failure status & potential chance of a ractionary hemorrage from the vascular blowout, it was advanced to undergo amputation at theappropriate level and this has to be performed before tomorrow.

User :   hi
Doctor :   Hi
User :   pls see the above issue and suggest
Doctor :   Let me read the query.
Doctor :   How can I help in this?
User :   whether we can go head with removal of the leg, or is there any solution
Doctor :   If there are gangrenous changes and if the vascular injury is severe and cannot be reversed, then the option is amputation.
Doctor :   Amputation is a live saving procedure here.
Doctor :   If a limb is gangrenous and is left alone, it is a potential source of infection, which will further compromise severely on life.
Doctor :   Hence this may have to be attempted.
Doctor :   Have you followed?
User :   i dont know this med terminology, but i have a brieg case sheet for u reference as a file how can i sent to you
Doctor :   Let me put it in simple words.
Doctor :   Limb undergoes changes when the blood supply to it is compromised.
Doctor :   It affects all the structures of the limb.
Doctor :   The changes that happen following this is referred to as gangrene.
User :   OK
Doctor :   Gangrene is a irreversible process.
Doctor :   The only treatment to this is removal of the affected part.
User :   OK
Doctor :   This is done basically to avoid the infections.
User :   To other body parts right ?
Doctor :   Hence, if there are gangrenous changes or examination reveals that the blood supply cannot be restored the treatment in such case is removal of the affected part.
Doctor :   Have you followed?
User :   thankyou
User :   bye
Doctor :   Do you have any other query?
User :   nothing sir
Doctor :   Thanks for consulting me. Bye for now. Please refer your friends to our service.
