User :   I have query regarding UTI

User :   the report is like this protein is +++, pus cells is 80-100/hpf, RBCs are 70-80/hpf and epithelial cells 3-4/hpf

User :   she had consulted a doctor before and he suggested a antiboitic sensitivity test after this test to suggest the proper antibiotic. now i have the report and she can not consult the doctor consulted before. so could you do something in this case

Doctor :   Now let me tell you few steps for this problem.
User :   ok
Doctor :   1. Does she have any fever ?
User :   no
Doctor :   2. Any other systemic symptoms ?
User :   she is having sensations in her uterus

Doctor :   Sorry to tell you that I am not supposed to prescribe antibiotics without seeing the patient physically and is aginst medical ethics.
Doctor :   But let me give some suggestions.
Doctor :   1. Dolo 650 mg tab. thrice daily for three days.
Doctor :   2. Drink lots of alkalysing agents like fresh fruits aand juices
Doctor :   3. Drink lots of water.
Doctor :   It is very easily treatable with one week course of antibiotics.
User :   she is sensitive to ampicillin/sulbactum, amikacin, amoxycillin, amoxycillin/clavunalic acid, ceftizoxime, chloralphenicol, ciprofloxacin, ceftazidime, cefaclor, cefotaxime, cefoperazome/sulbactum, doxycycline, gentamycin, gatifloxacin, imipenem, levofloxacin, netilmycin, nitrofurantoin, ofloxacin, piperacillin/tazobactum
Doctor :   Hope you have followed, Dehydration is one big reason for UTI.

Doctor :   I suppose we can try with many other antibiotics like Norflox or tinidazoles.

User :   she is moderately sensitive to tetracycline she is resistant to CO-trimoxazole, cephalexin, nalidixic acid, norfloxacin

Doctor :   How about Glycopeptides and Lincosamides.
Doctor :   Like Vancomycin or Clindamycin ?
User :   she has no problem with that
User :   she was taking dalaforce(balofloxacin) tablets and alkasol syrup

Doctor :   So lets say she is broadly sensitive to 1. Aminoglycosides ; 2. Cephalosporins ; 3. Pencillins ; 4. Quinolones ; 5. Sulfonamides ; 6. Tetracyclines , BUT there are numerous other antibiotics which can be tested on her with low doses and then given for treatmnt.
Doctor :   As I have said earlier I can suggest that there are numerous other Groups she can be non-allergic or non-sensitive to .
User :   she has no problem with vancomycin or clindamycin as per report
Doctor :   But I don't want to prescribe you a medication without seeing the patient as it is against medical ethics.

User :   you suggest a proper antibiotic and i would say she is sensitive to it as i have the report
Doctor :   Please try to understand, we doctors have to follow medical ethics that we take an oath . I can tell you as many group names and name some antibiotics but cannot give you the dosage or prescription without examining and testing the patient with the antibiotic.

Doctor :   3. Polymyxin B
User :   then will you please suggest a general non-harmful medication as earlier you suggested dolo tablets

Doctor :   4. Vancomycin
Doctor :   3.Polymyxin B
Doctor :   5. Clindamycin
Doctor :   6. Lincomycin
Doctor :   Or even 7. Erythromycin class Macrolides
User :   so what do you propose right now. what would be the proper action
Doctor :   Hope you understand our constraint. And take help from an Experinced Gynecologist onthis who does physical examination and then treats her.
User :   she was given balofloxacin tablets by the previous doctor
Doctor :   How was her sensitivity to this medication.
User :   she does not feels good after taking it
Doctor :   Ok
User :   you have two minutes to suggest if you want to suggest something. my time is over
Doctor :   So the Gynecologist can try with the Erythromycin group or Macrolides or with Vancomycin which do not cause much sensitivty or adverse reactions .
Doctor :   Even Clindamycin might be helpful.
Doctor :   Please do this at the earliest otherwise may lead to PID
