User :   Hi, I have consulted a Nephrologist for my wife and after having urine and blood tests doctor is recommending Biopsy as too much of protein is being released from her body (253 mm, when an avg should be 0-11 mm). also her BP level is too high even after having 2 stamlo tabs everday. But I am concerned if we should opt for this. Pls advise
Doctor :   Hi
User :   hi
User :   her age is 22
Doctor :   I will help you. please give me a couple of mins to read the query.
User :   ok
Doctor :   Since her protein is high
Doctor :   we need to evaluate this further
Doctor :   any problem in the kidneys will cause these chages
Doctor :   it could be acute or even a chronic cause
Doctor :   the reason for the biopsy is to find what changes are taking place in the kidney
Doctor :   so, I will also suggest you to go for it.
User :   will there be a pain in doing so?
User :   as my wife is against doing this
Doctor :   pain will be minimal, the pain will be taken care of.
User :   if any side affects , pls advise?
Doctor :   but this test is very essential
Doctor :   theywill give her a local pain killer, so she will not feel the pain.
User :   side affects?
Doctor :   without biopsy it is tough to come to a conclusion.
Doctor :   this is a diagnostic test
Doctor :   it is associated with very minimal complications
Doctor :   being bleeding and pain.
Doctor :   she need not fear so much
User :   ok, should she be hospitalised ?
Doctor :   it is a small procedure for about 30 mins to an hour.
Doctor :   she may need a day's admission.
User :   doc says 2 days to keep her under observation
Doctor :   it depends on the doctor.
User :   what is cultural sensitive test?
Doctor :   yes, as I said, it depends on the patient's condition and the doctor's decision.
Doctor :   culture sensitive test is done on urine, to see the micro organism growth in the sample
User :   bcz we are awaiting for this test reports for CST
Doctor :   and to detect the specific cause for the infection
Doctor :   this test reports will take 48 hrs or more.
Doctor :   yes, you will be asked to wait.
User :   yep, it was given on last Saturday and expecting them to come today
Doctor :   they need to grow the organisms and find out to what medicines they will be sensitive to.
Doctor :   so that they can start right treatment.
User :   if cul sensitivity is positive , do you still suggest to take Biopsy to check just loss of protein?
Doctor :   let me tell you, biopsy here is very much needed
Doctor :   even if there is infection as the cause
User :   ok
Doctor :   it can lead to changes in the kidney function.
Doctor :   by doing biopsy, we will know what exactly is leading to the protein loss, it could be infection complicating further
User :   just to let you know her father and sister had the same issue and diagnoised
User :   is it family related?
Doctor :   what were they diagnosed with
Doctor :   do you have the final diagnosis?
User :   not sure but they had kidney problems
Doctor :   many conditions of kidney is family related too, but then we cannot come to any conclusion right now.
Doctor :   only after the relevant tests,we can compare and comment on it.
User :   ok, if it's family related, will it also affect to our future generation?
Doctor :   as I told you , if we get the reason for her problem , we can suggest what will be the effect on your children, but if it is due to an infection , it will not get inherited
Doctor :   it is very early to comment on this.
Doctor :   biopsy is the first step
Doctor :   to proceed further, then on she will get the right treatment.
Doctor :   not all diseases necessarily have to be present in children
Doctor :   they need not be also even if she has the condition.
Doctor :   are you with me?
User :   ok, after having Biopsy result will my Nephrologist will able to comment on affect on the future generation?
Doctor :   yes, he will surely be able to get a clear picture
Doctor :   you need not worry about it.
User :   is this 100% curebale?
Doctor :   as not all of them will be passed on,it could just be inherted and get cured later and managed.
Doctor :   We cannot say about 100 percent cure, as we have no final diagnosis, so far the tests are only suggesting there is some problem with her kidney.
Doctor :   once we get the biopsy report, we can rule out many possibilities and see the stages and severity of the problem
Doctor :   based on that, we can tell the treatment and prognosis of the same.
User :   ok, any idea abt Biopsy charges ?
Doctor :   It depends on the Nephrologist charges.
User :   I mean approx?
Doctor :   it varies actually.
Doctor :   anywhere between 1200to 2000 rs
Doctor :   it could be more also.
Doctor :   it depends on the doctor, hospital and procedures, may be they are looking for a CT guided biopsy
Doctor :   there are different types of procedures
Doctor :   to do a biopsy
User :   ok
Doctor :   if it is ultrasound guided it may be lesser, but if it CT guided may cost so much
Doctor :   depending on what they are looking for , as in the best type to helpin diagnosis will be chosen.
User :   k
Doctor :   did you understand what i suggested?
User :   after getting the Biopsy report, can I possibly email to you and seek your advise?
Doctor :   yes , sure
Doctor :   we have specialists on our panel
Doctor :   you could chose from them , mail us your questions
Doctor :   enclosing the reports
Doctor :   on
Doctor :   you could also contact us for a phone call query
Doctor :   where in you will be connected to our specialist and can ask your doubts
User :   one lase question on her BP
Doctor :   yes, please go ahead.
User :   she is having 2 stamlo 2 tabl but getting no progress
Doctor :   we must check what is causing her BP
User :   also the doc has added a new tab somthing called maxpress 2.5
Doctor :   yes, she can take it along
Doctor :   a combination of medicines will be added
User :   will Biopsy also tells the reason for high BP?
Doctor :   to control her blood pressure
Doctor :   yes, it will tell us
Doctor :   generally the contributing factors will come to light, in a biopsy
User :   ok then...
Doctor :   if we treat the cause, her bood pressure can be controlled
Doctor :   but she should not stop her BP medicines.
User :   how long?
Doctor :   the dose and changes will be made by the doctor
Doctor :   she has to continue them, constant monitoring has to be done.
Doctor :   she may need it probably for ever.
User :   that's bad
Doctor :   as blood pressure has to be controlled , but she may be put on a lower dose.
Doctor :   we can decide this after her evaluation.
User :   k
Doctor :   then only we can tell you about medicines as well
Doctor :   please proceed with the test.
Doctor :   as advised
User :   ok, thanks for your suggestions
Doctor :   did i answer all your questions?
Doctor :   any more questions for me?
Doctor :   thanks for consulting m
Doctor :   me
Doctor :   bye for now .
